Care is moving beyond hospital walls
Health services across Australia are delivering more care across hospital, home and community settings. This shift is changing how clinicians coordinate assessments, documentation and communication between teams. Care no longer sits neatly within one ward or one service.
The growth of virtual care, remote monitoring and Hospital in the Home programs has introduced new handovers, visibility requirements and ways of sharing information across teams.
COVID accelerated this transition. Services moved quickly to scale telehealth and out-of-hospital programs. What began as an immediate response is now becoming part of routine care delivery.
Hospital EMRs and care beyond the ward
Hospital EMR systems have developed over many years to support inpatient care. They are designed to manage admissions, documentation and clinical workflows within hospital settings.
Today, those systems are increasingly being used to support care delivered outside the hospital. As out-of-hospital programs expand, health services must consider how information flows between teams, where documentation occurs and how coordination is maintained across settings.
Integration makes information accessible, but it does not automatically define how care is coordinated across teams and environments. Clear workflows, defined responsibilities and appropriate governance become increasingly important as these programs grow.
From integration to scalable care
Connecting systems is an important step. Designing care models that work within those systems is the next challenge.
CareMonitor is an Australian digital health platform supporting coordinated models of care across hospital, home and community settings. The platform is used by health services delivering programs such as Hospital in the Home, virtual care and chronic disease management, helping teams coordinate care pathways, share information and align workflows with existing EMR environments.
As EMR programs continue to mature, the focus is shifting from connecting systems to designing models that operate reliably across settings.
Integration lays the groundwork. Sustainable scale depends on clear workflows, defined roles and appropriate governance built around it.
Join us at WA Digital Health Summit 2026
Designing scalable care models in an EMR-connected world.
The session will explore practical insights from operational environments and examine what influences whether connected care models can operate effectively at scale.
We look forward to continuing the conversation at the Summit.
