Country to Coast, QLD

Central QLD, Wide Bay, Sunshine Coast PHN is an independent, not-for-profit organisation and we deliver the Primary Health Network (PHN) programs. February 2023 marks an important chapter in our organisation’s history as we proudly announce our new name, Country to Coast, QLD.

Our Vision is to support our region to live their healthiest lives and our Mission is to work with our community to deliver leading primary health.

CCQ works alongside community members and health professionals to improve access to well-coordinated quality health care. Collaborating with our partners in the state and federal government and the non-government sectors, we endeavour to reduce health inequalities for our region.

CCQ has seven key priorities for health improvement: mental health, alcohol and other drugs, Aboriginal and Torres Strait Islander health, population health, health workforce, digital health and aged care

We are CCQ, a diverse team of dedicated people who work collaboratively with each other, our stakeholders, and the community. We’re motivated to achieve one shared goal: improve health outcomes, from Country to Coast.
